How To Apply For Assistance?

Applicants must be 18 years of age or older to apply for assistance. Submission of this application is not a guarantee of acceptance into the program. All applications will be reviewed within 7 business days, and a member of our team will contact you to let you know if your application has been accepted. Before submitting your application in-person or online, please ensure that you have ALL of the required paperwork. If paperwork is missing, your application may be denied. If your application is approved, you will be responsible for picking up your donations once they are available. We are no longer able to hand-deliver your items. Please note that NO MONETARY DONATIONS WILL BE GIVEN.


Paperwork Needed To Apply:

  1. Photo ID
  2. Proof of current physical address, if different from your photo ID.
  3. Provide Income sources for everyone in the household.
    • If you are currently living with someone other than immediate family (free or renting), please have them provide a written letter stating your current living conditions. Their income will not be required for application.
  4. Proof that you are the parent or legal guardian of each child you are registering.
    • Document(s) must have your name and the name of each child you are registering.
    • Forms of documents that are accepted include birth certificates, court orders, and notarized/judge signed order.
  5. Proof of child’s age
    • Forms of documents that are accepted include birth certificates, shot records, and school records with the child’s date of birth clearly listed.
  6. Completed application form
    • A copy of the application form can be found below.
    • You must submit one parent sheet per family, one child sheet per child, and one signature sheet per family. If you are applying for more than one child, please make sure all papers are kept together.
